	Rider Application Form
	This form you need in order to partake in the bike ride as a rider. Bring it filled out to the sign-up at Siam Bayshore, the beach entry to the hotel, or visit Shenanigans Saturday 15th September. Between 6 and 9pm they accept registration. More than one rider can be registered on one form.
	Main Sponsorship Form
	With this form you are collecting sponsorships from your friends, family members, collegues, strangers... anybody who is willing (and trusts you!) to give you ANY amount of sponsorship. This form together with the money and the Rider Application Form will be handed over on sign-up either at Siam Bayshore on the event day or the evening before at Shenanigans. If you run out of lines for your sponsorships, you can print out the next form - the Additional Sponsorship Form.
	Additional Sponsorship Forms
	This Form is an addition to the above form. If you have lots and lots of people sponsoring you might prefer to print out this form with more lines than the above. Now we wish you successful collecting and hope you have as much joy doing the good deed as the kids will have who will profit from your efforts in the end.
